Idaho Speech-Language Pathology Assistant License Reciprocity Process

Individuals practicing as SLPAs in other states can gain licensure in Idaho through an endorsement process. The licensure requirements in the original state should be similar to the requirements in Idaho. Send your notarized application in with the following items:

  • Completed open-book exam
  • Government-issued ID
  • Name change verification (if applicable)
  • Verification of Licensure from issuing state(s)
  • A copy of the original state’s licensing rules and regulations (if requested by the Board)
  • $95 fee (includes $25 application fee and $70 license fee)

The Board will send a notification of licensure to your address on record once the credential is issued.
