Utah Speech-Language Pathologist License Renewal Procedure

SLP licenses are renewed every two years online or by completing the paper application. May 31st of odd-numbered years is the date of expiration for all SLP licenses. About 60 days before the licenses are set to expire, a renewal notice is mailed to the address the DOPL has on record, so be sure it stays updated.

When renewing online, you can use a credit or debit card for payment. Using the online system saves processing time as the updated license is usually emailed to you the following business day.

For paper renewals, review the checklist below before mailing your paperwork:

  • Answer each certification question and provide documentation if needed
  • Sign the Affidavit or submit an explanation as to why you cannot
  • Sign checks and money orders and make them payable to the DOPL
  • Submit legal name change documents if applicable

Licensure renewal also requires a minimum of 20 hours of continuing professional education (CPE) during each biennial cycle. The CPE must be directly related to speech-language pathology or relevant to the practice. Instructors should be qualified to provide training in a competent and sequential manner.

CPE hours can be gained through employer-sponsored in-services, ASHA-approved providers, and Utah Speech-Language-Hearing Association (USHA) sponsored activities. Business and management courses that will help you run your private practice much more efficiently will also be accepted toward your CPE hours. You are responsible for keeping records of the CPE you’ve completed for at least four years.
