Arizona Speech-Language Pathology Assistant License Reciprocity Process

License reciprocity is a process in which individuals holding a license in another state or US territory can become licensed in Arizona without retaking the national exam. To qualify for reciprocity, the license must be valid and at least a year old.

The application fee of $100 is nonrefundable, and you’ll also need to pay the $200 license fee. You may qualify for a fee waiver if your household income doesn’t exceed 200% of the federal poverty guidelines, you are the spouse of an active duty military member, or are an honorably discharged veteran who was discharged no more than two years before the date of the application.

Complete the application and send it to:

Bureau of Special Licensing
150 North 18th Ave
Suite 410
Phoenix, Arizona 85007

It typically takes the Department 30 days to process the application and issue the SLPA license.
