Hawaii Speech-Language Pathologist License Reciprocity Process

Individuals licensed in another state or certified by ASHA can apply for licensure by reciprocity. Upon moving to the state, submit an application and you may begin practicing for a period not to exceed 90 days if you meet the requirements mentioned in the previous sentence. The Board will process the application and issue a license if your existing license meets Hawaii’s educational requirements.

Send the License Verification Form to the issuing state to complete. The paperwork will then be mailed to:

You also need to submit the following to the Board.

  • Clinical Observation and Clinical Practicum Verification
  • Clinical Fellowship Verification
  • Verification of Coursework
  • Official transcripts
  • Three professional letters of recommendation
  • Praxis exam score
  • $264 processing fee for licenses issued in even-numbered years
  • $176 processing fee for licenses issued in odd-numbered years
